Dear Food Establishments and Tobacco Permit holders,
The Webster Health Department has decided to extend your current 2020 Food/ Tobacco Permits until November 1, 2020 due to the hardships of COVID-19.
We will be accepting all Food/ Tobacco Applications by mail or drop box, located near the parking lot at the Town Hall.
If you require assistance with your application, please call the office and we will be in touch with you as soon as we are able.
Please submit your application with ALL required documents attached no later than October 15, 2020.
Reminder of required documents for Food Establishments-
- 2 Food Managers Certifications minimum
- Choke Savers (cpr) someone on every shift
- 2 Allergen Awareness
- copy of liability and workmen’s compensation insurance
- updated menu
For Tobacco Retailers
-tobacco a copy of MA sales tax and use certificate for tobacco
- liability insurance
- workmen’s compensation insurance
Please note: Incomplete applications will not be processed.
Also, food/ tobacco inspections will still continue as required.
