[ARCHIVED] COVID-19 Vaccine Enrollment


The questions on the survey will help us determine in which phase you are eligible for the vaccine. As the vaccine becomes available, in accordance with the Commonwealth of Massachusetts' Phased COVID-19 Distribution Plan, enrollees will be contacted by the Board of Health and provided with the instructions on how to receive the vaccine. Residents that do not have the ability to access the internet are encouraged to first ask for assistance from family members, caregivers or friends who will be able to go to and complete the form on their behalf.  If you are inquiring about several individuals, please fill out one form for each individual.  The entire enrollment process takes less than five minutes. Residents can also come into Town Hall Monday through Thursday 9:00 AM to 4:00 PM to complete the survey or call 508-949-3800 ext 1029. If calling on the phone, you may need to leave a message--you will still be enrolled.

The Commonwealth has also developed a website and interactive map for residents to sign up for the vaccine. The link is below. Webster residents may also seek to register for the vaccine at through this site. Individuals 75 or older can begin making appointments for dates February 1 or later. However, due to high demand and constrained vaccine supply, COVID-19 Vaccination appointments are limited and appointments are difficult to schedule. More appointments will be available based on supply from the Federal Government. Appointments will be added on a rolling basis. Individuals must present proof of their eligibility to receive the vaccine. Some sites require individuals to attest to their eligibility as part of the online appointment scheduling process. All other sites will accept the Commonwealth’s Self Attestation form, which can be filled out online. Residents should be prepared to present this form in a hard copy or electronically at their appointment.
